- Any contact lenses you are currently wearing
- List of of any medications you are currently taking
24 Hour Cancellation Policy
Please inform us at least 24 hours prior to your visit if you need to cancel or reschedule your appointment. This allows us to offer this appointment slot to other patients who may have an immediate need to our care.
There is a $50 charge for all cancellations or no-shows that occur within a 24 hour period. This is a financial necessity to compensate the doctor for the loss of revenues incurred by these scheduled appointments.
